A heinous horn-swaddler snaps his chain and charges at you.

The heinous horn-swaddler looks like a mutated bugbear.  This humanoid has bat-like ears, a squashed nose, and a sharp-toothed grin.  Three straight horns thrust from the top of its skull.  The middle one is at least a foot long!
